Resource Purpose
Non-Public School Staff - Student Data Export Template             

This template can be used to export the data required for our service providers to prepare 407-1s for eligible students.  They will need this data provided for all eligible students.

Child Study Team Resources

Resource Purpose
Non-Public CST Referral Form To refer a student for a Child Study Team evaluation determination.  The information in this form will request required documentation and will automatically send the Parent/Guardian CST form to the parent/guardian.
